Requirements for Companies

The requirements shown here are for companies interested in submitting mutilated coins to the Mutilated Coin Redemption Program.

In addition to the information listed on the Instructions page, companies need to include the following information with their applications:

  2. Parent Company Information (if applicable)

Include all of the information listed below with your application.

  • Form of Ownership
  • In which year was the company established
  • Tax ID Number or Employer ID Number
  • Street Address (street number and name)

  • Country*
  • Company Phone Number
  • Company Website
  • State where registered (if applicable)
  • Registration Number (if applicable)

*Note: Participants shipping coins from outside the United States in an aggregate dollar amount exceeding $10,000 are required to submit the Report of International Transportation of Currency or Monetary Instruments (CMIR), FinCen Form 105.

Parent Company Information

If the company is a subsidiary of a parent company, the information listed below is also required with the application.
